TRAVELMED TERMS OF SERVICE, TELEMEDICINE CONSENT, AND PRIVACY POLICY
PURPOSE
TravelMed LLC provides services intended for United States residents in states where its contracted providers are licensed. TravelMed facilitates access to telemedicine review by U.S. licensed physicians related to pre-travel prescription medications (“Rx Kits”), which may be medically appropriate for anticipated travel to remote or resource-limited areas.
By using the website or purchasing any TravelMed services or products, You agree to the terms outlined below.
DEFINITIONS
TravelMed LLC (“TravelMed,” “We”) – the service platform
Patient (“You”) – the individual or group purchasing or using services
Licensed Provider – an independently contracted U.S. licensed physician
Partner Pharmacy – a third-party licensed pharmacy responsible for dispensing medications
Rx Kit – TravelMed service including telemedicine review, organization materials, and prescribed medications

TERMS OF SERVICE
GENERAL TERMS
TravelMed provides a platform that connects Patients with independently licensed physicians for telemedicine review related to travel medications.
A patient-provider relationship is not established until a Licensed Provider has reviewed the Patient’s information and has agreed to provide care.
Licensed Providers are responsible for all medical decision-making and exercise independent clinical judgment. Providers are licensed in the state where the Patient is physically located at the time of the consultation, as required by law.
You agree that You will be physically located in a U.S. state where the Licensed Provider is authorized to practice at the time of telemedicine review.
Telemedicine services may include review of submitted health information, medical history, and travel plans to determine whether prescription medications are appropriate.
You are responsible for providing complete and accurate medical information. TravelMed and its Licensed Providers are not responsible for outcomes resulting from incomplete, inaccurate, or misleading information provided by You.
There is no guarantee that any prescription will be issued.
TravelMed services are intended for individual patient use unless otherwise specified in a separate written agreement for group or organizational use.
RX KIT PROCESS
TravelMed facilitates telemedicine review by a Licensed Provider. If the Provider determines that prescription medications are appropriate, prescriptions are transmitted to a Partner Pharmacy for fulfillment.
All medications are dispensed and shipped directly to the Patient by the Partner Pharmacy in accordance with applicable laws.
TravelMed does not manufacture, store, dispense, or directly distribute prescription medications.
Medications are included as part of the Rx Kit service pricing.
Medication availability, selection, and suitability may vary based on medical judgment, pharmacy supply, and regulatory limitations. TravelMed does not guarantee that any specific medication will be prescribed or available.
FOLLOW-UP TELE-SUPPORT
TravelMed Rx Kits include limited non-emergent follow-up tele-support for general medical or medication-related questions while traveling, including in remote or resource-limited settings.
This support is limited to approximately one (1) cumulative hour over a one (1) year period from the date of purchase.
This service is not intended for urgent or emergent medical situations. Response times and availability are not guaranteed.
ANNUAL RENEWAL
TravelMed Rx Kits are designed as an annual service. Renewal may include updated prescriptions, medication refills, and continued access to tele-support, subject to a new medical review by a Licensed Provider.
Prescriptions are issued at the discretion of the Licensed Provider and are not guaranteed upon renewal.
CANCELLATION AND REFUNDS
TravelMed offers a customer satisfaction guarantee. Patients may request cancellation and a refund at any time by contacting TravelMed.
TravelMed reserves the right to review refund requests and to deny refunds in cases of misuse, abuse, or violation of these Terms.
LIMITATIONS OF SERVICE
TravelMed services are intended for episodic, travel-related medical support and non-emergent situations only.
TravelMed does not provide emergency medical care, emergency telemedicine services, medical evacuation services, or any form of insurance.
If You are experiencing a medical emergency or acute symptoms, You should seek immediate in-person medical care.
Use of any prescribed medication does not replace the need for appropriate medical evaluation when indicated.
TravelMed services are not intended to provide ongoing, primary, or chronic medical care.
NO GUARANTEE OF OUTCOMES
TravelMed makes no guarantees regarding medical outcomes, effectiveness of treatment, or prevention of illness.
All services and products are provided on an “as-is” basis.
ELIGIBILITY
Services are intended for individuals 18 years or older.
Minors may only receive services through a parent or legal guardian who provides consent.
PAYMENT AND PRICING
TravelMed sets pricing for its services and Rx Kits at its sole discretion.
All payments are cash-based. TravelMed does not accept or bill insurance.
NO RESALE
Rx Kits and services are intended for end-user use only and may not be resold or redistributed.
RULES OF USE
You agree not to misrepresent identity or medical information, use the service for unlawful purposes, interfere with platform operation, or circumvent provider oversight.
TravelMed reserves the right to suspend or terminate access for misuse.
THIRD-PARTY SERVICES
TravelMed works with third-party providers including pharmacies, payment processors, and service vendors.
TravelMed is not responsible for the actions, services, or outcomes of third-party entities.
DISCLAIMER OF WARRANTIES
All services and products are provided without warranties of any kind, whether express or implied.
LIMITATION OF LIABILITY
To the fullest extent permitted by law, TravelMed is not liable for any indirect, incidental, or consequential damages arising from the use of its services.
Maximum liability shall not exceed the amount paid for the service.
INDEMNIFICATION
You agree to indemnify and hold harmless TravelMed and its affiliates from claims arising from your use of services or violation of these Terms.
TERMINATION
TravelMed may suspend or terminate services at any time for misuse, non-payment, or violation of these Terms.
GOVERNING LAW
These Terms are governed by the laws of the State of Hawaii.
FORCE MAJEURE
TravelMed is not responsible for delays or failures due to causes beyond its reasonable control.
TravelMed operates in compliance with applicable state and federal laws governing telemedicine and prescription services.
PATIENT CONSENT TO TELEMEDICINE
INTRODUCTION
Telemedicine involves the use of electronic systems to allow Licensed Providers to review Patient information and provide medical guidance.
This may include review of medical history, symptoms, and travel plans.

MPORTANT LIMITATIONS
Telemedicine does not replace in-person care, may not be appropriate for all conditions, and may be limited by incomplete information.
Licensed Providers will determine whether telemedicine is appropriate.
RISKS
Potential risks include incomplete or inaccurate information, misinterpretation of symptoms, delays in care, and technical or communication limitations.
PATIENT RESPONSIBILITIES
You agree to provide accurate and complete information, follow medical advice given, and seek in-person care when appropriate.
CONSENT
By using TravelMed services, You consent to telemedicine review, understand the limitations and risks, and acknowledge that no guarantees are made.
PRESCRIBING
You understand that prescriptions are not guaranteed, no controlled or addictive medications will be prescribed, and all prescribing decisions are made at the discretion of the Licensed Provider.
PRIVACY POLICY
INFORMATION COLLECTION
TravelMed may collect personal identifying information, health-related information, and transaction and usage data.
USE OF INFORMATION
Information may be used to provide services, facilitate telemedicine review, process transactions, and improve services.
SHARING OF INFORMATION
Information may be shared with Licensed Providers, Partner Pharmacy, and service providers necessary to operate the platform.
TravelMed does not sell personal health information.
SECURITY
TravelMed uses reasonable safeguards to protect information but cannot guarantee absolute security.
DATA RETENTION
Medical records are retained in accordance with applicable laws and regulations.
USER RIGHTS
You may request access to your information or updates as permitted by law.
COMMUNICATIONS
TravelMed may send service-related communications. Marketing messages may be opted out of.
